ATSA Membership Application

  1. The application process for ATSA requires the completion of the Membership Application and payment of the application fee. Please download and complete the Membership Application, which is emailed, faxed or mailed to the ATSA office.

  2. If you are not licensed or certified, then you must submit two Letters of Reference with your application. Letters of Reference from a supervisor or colleague who is familiar with your work are required. Please download the Letter of Reference form and email, fax or mail a copy to each of your references for completion. Your references can email, fax or mail the form and letter directly to the ATSA office. Students need only to submit one letter from their academic or field supervisor. A copy of your academic transcript must also be submitted.

  3. If you are licensed or certified, a letter from your licensing or certification board(s) is required. If your licensing board has online license verification available, written verification from them is not required. If online verification is not available, please download and complete the Licensing/Certification Form to be printed and mailed to your board(s). One Letter of Reference is also required if you are licensed or certified. A photocopy or scanned copy of your license(s) must also be submitted.

Incomplete applications cannot be processed; however, all supporting documents received in the ATSA office will be kept on file until the application is complete.
