The Harbor Point Case Manager - Clinician is a key associate at Hope Network whose primary responsibilities include: Coordinating all services necessary to maximize individual safety, wellness, and independence. This position provides clinical services in an intensive setting with individuals who typically have experienced long periods of institutionalization.
The Harbor Point Intensive programs offer specialized residential treatment for a individuals experiencing mental health and behavioral concerns, and who require a higher level of support and supervision. Individuals served at Harbor Point often have histories of problematic sexual behaviors and/or assaultive behaviors. Residents may also have lengthy histories of psychiatric hospitalizations and/ or criminal histories resulting in periods of incarceration. Assessment of risk and recidivism is conducted prior to placing anyone in the Problematic Sexual Behaviors group, which embraces the Good Lives Model. Psychoeducational groups and therapy groups offered include Dialectical Behavior Therapy (DBT) skills, Acceptance and Commitment Therapy (ACT), Anger Management, Substance Abuse, Cognitive Behavioral Therapy, Seeking Safety, and Social Skills. All services are provided with a trauma-informed approach.
The Harbor Point Case Manager-Clinician is responsible for providing case coordination, as well as group and individual therapy. This position participates in a multidisciplinary team committed to serving individuals with high risk and high needs to achieve greater levels of independence.
Educational / Talent Requirements:
- Minimum Education required: Master Social Work Degree. Licensed Master Social Work (LMSW) or Limited Licensed Master Social Work (LLMSW) or Licensed Psychologist (LP) or Limited Licensed Psychologist (LLP) or Temporary Limited Licensed Psychologist (TLLP).
- Current Michigan driver's license with acceptable driving record according to Hope Network insurance carrier definitions.
Work Experience Requirements:
- Prior clinical experience in a mental health, correctional, or rehabilitation setting preferred.
